On back, when folded, summary of conditions of tenancy, 1 - 14. Inside, left (green): Printed lines for name and address of tenant. When open, columns for date (April 1961 - March 1962), rent, arrears, allowances, cash paid, collectors initials, and arrears due, on two right columns. Left column blank except for note re: differential renting. Tenants name above right.
