Ps2-3 for names of lives insured, ps4-5 with printed calendared weekly contributions in imperial currency to 12 Feb 1971, entered in biro, ps5-13 for decimal currency. Ps14 to back cover printed extracts of Insurance Act 1936 etc. Contributions 2s (10p) per week beginning 2 Oct 1970 and continuing to 14 Sep 1979 (end), paid generally weekly.
