Single sheet folded printed black on white on one side and with red and blue on inside. Printed by Hely's Ltd Dublin. Watermark of fleur de lis in cartouche and "Swift Brook/ Unique/ Ledger". Filled out in black ink and signed by P.V. Twomey and Joseph A Susher.
