Printed black on white, and made out in black ink. Front has the details of the payment and is stamped "This is not to be taken as a receipt" in red, and typed ? in blue that the collector will be in Croom on Thursdays 1,8 & 15 Sept.; name & address of collector stamped in red, "G.E. Farmer/ Barrington Street/ Limerick". Printer's name at bottom left at front "Quinnell & Sons, Printers, Tralee. Reverse haslist of charges and extracts from the relevant act. Some pencil additions on the reverse. Remains of perforations down left side when viewed from front.
