Prescription docket. Mid-Western Health Board docket, with circular perforation either side at top; printed black on white with spaces to be filled out for date, name, cost etc; docket has remains of perforation across top and bottom edges. Affixed to the centre is a label printed in green and blue on white, filled out by computer in black. label reads "Keep out of reach of children/ L. SLoan B.Sc (Pharm) M.P.S.I. [green cross]/ Castleconnell Pharmacy, Co. Limerick. Ph.377494". Filled out 14-2-96 with prescription for Lenore Walsh, from Dr. P. Murray. Rubber stamped with rectangular stamp of the Pharmacy.
