All pages have the City Arms and motto at centre top; with below left "Memo from/ J.J. Forrest/ City High Constable/ Sanitary Officer/ Inspector of Food and Drugs, Bakeries, Shop/ Hours, Hackney Cars &c" at left and to right "Town Hall/ Limerick 191 / To.........../.........../ Ashe Printer Limerick". Printed black on white with blue lines below the head. Watermark "Ballyclare/ Co. Antrim". Filled out in ms 2-9-1914 to the Chairman and gentlemen of the Public Health Committee, annotated in red. 7th page rubber stamped and signed that correct by J.J. Peacocke. Annotations in red.
